Cardinal Glennon doesn't have oral surgery. They suggested another large local hospital chain. They do have oral surgery but through a private practice and don't accept Medicaid. I cried on the phone to their intake person and begged for her to ask for an exception and/or cash payment. She's supposed to ask and get back to me. She also suggested 3 other places which I already called and was turned down. I've added Medicaid ombudsman to the call list. Just too broken to make any more calls right now.

According to Google, 42% of the US population is obese. I am not suggesting anesthesiologists should ever take risks, but I do find it curious that there are not procedures in place to mitigate the dangers of anesthetizing obese patients. Could you call back and say that you understand they were not prepared to deal with BT that day but could they put a plan in place knowing that she is high risk? It was a firm no and closed door. Betting it's a combo of BMI and her seizures and they are covering their ass. Completely stupid that it took them over a month to do so. Def appreciate the brainstorming though so keep it coming. Teen spent the night crying in her sleep which I think is worse than seizures. I hate, hate, hate making phone calls and also hate crying in front of people, so this is like my worst nightmare combination but I'll keep toughing it out bc I love her.

DH and I are frustrated with DD1. Since 2nd grade, she’s been in magnet programs that we’ve needed to take her to and from school for as they aren’t her home schools. The elementary school was close to our house so not that much out of the way while her middle school is definitely out of our way. I take her in the morning and DH picks her up.
She’s in 8th grade this year (she’s 13) and there’s a specific magnet program she REALLY wants to get into. There’s an application she needs to complete to get in. Part of that application is answering a couple of questions about why you want to go there along with achievements, etc. for the program. The application is due the 1st week of December. She submitted the application yesterday afternoon without letting DH or I look at to review what she wrote. DH is livid while I am too. She does have a portfolio to submit too and that’s due the 1st week to January.
We are afraid that she won’t get in as she’s not a detail oriented person in some ways that we see in everyday life. She does get straight As; however, we have been hearing a lot more of not doing great on tests initially then redoing the wrong ones to get your grade better.
I’m aware she’s 13 and displaying her independence but we are afraid this will bite her in the ass and she won’t get in. She’s also about to get a CTJ moment on mom won’t be a pushover anymore. If I say to do something and it doesn’t get done, it will be time to face the consequences and she won’t like them.

BT likely would not be going with a student dentist, but either one of the dentists going for advanced training or the faculty. It all depends on whether the school has a residency program or not for specialties. I think that there are still a few schools out there that do not have one.

When we initially tried to sell our first house the sale fell apart and we had done some work on the house we were purchasing just to get the house past inspection. We ended up eating the $$ we had invested and letting the purchase fall apart. What is causing you problems?

My dad paid for my brother's oral surgeon in cash so Bob could get his tooth removed. Finding a Medicaid dentist that would do it was damn near impossible. It's why judging people concerning their teeth is wrong. Being able to afford to have clean well taken care of teeth is a privilege in this country. Now I'm confused. The anesthesiologist won't "put her under" because she's obese and has seizures or because she is on Medicaid? the anesthesiologist won't put her under because she is obese. But the reason Azcuena just can't hop over to another clinic is she has to find someone who will accept Medicaid. I have lots of in network people to shop from. Since I have a PPO I don't have to worry about having recommendations/referrals for the most part I can see whoever I want. I can probably find a dentist that will remove my tooth with relative ease and is in network. If not I can probably find one to dangle a cash payment in front of and bypass my insurance entirely. Someone on Medicaid does not have those luxuries. Being healthy is a privilege especially in America where it is all tied to having a job and hospitals are for profit institutions.

I’m not saying it’s right, but in a sue happy US, it means doctors are not going to take risks that they do not want to take. Add to this that some insurance companies WILL NOT cover surgery over a certain BMI.

She’d likely have these issues if she wasn’t on Medicaid, but by being on Medicaid there are a lot less places to go.

I'm thinking the issue is they won't put BT under in a clinic setting due to BMI. They have less resources there if something goes wrong and undoubtedly BMI is a number the insurance/lawyers have decided on.

I'm in St Louis. This is the original dental school that we went to multiple times. The last time I could hear teen and others screaming in pain from the waiting room. The dentist supervisor said I gave her 10 numbing shots and she just keeps feeling pain. FWIW, neurology completely signed off on seizures not being an issue once she's under anesthesia. I've seen the notes. The obesity concern is valid but this oral surgery office saw teen in person 8/23 so they've known this roadblock since then. I suggested to them yesterday that they change their process if they are going to deny obese patients care to do so right upfront. This oversight cost teen so much time, energy and pain. Today is the fourth missed school day this month that I'd tie directly back to tooth issue instead of mental health. Meanwhile social worker called this morning bc bio nut is making waves that I've made this all way too complicated and we should just go back to original medicaid dentist that we saw in April who said tooth is so messed up that they wouldn't touch it. So I had to explain teen's history yet again and say refer to the medical timeline that I provided you 3 weeks ago with everything tooth related highlighted in red. I asked social worker if she had any ideas who else might provide oral surgery and she said not her job. Okay, but is there someone within your department who can help? Nope. Even if I can find cash pay, bio is likely to cause problems. It's like a giant neverending clusterfuck.

While the dental clinic may have been ok with it, anesthesia is where the roadblock is and usually anesthesia is a separate person altogether, frequently brought in specifically for this under contract. I agree with you, this shouldn’t be this way. But I have heard dozens of stories where once anesthesia gets the records, surgery gets canceled due to BMI. The crappy part of this is that anesthesia frequently isn’t brought in until the very end, then everything comes to a screeching halt. Every appointment I have had with anesthesia has either been the day of the surgery, or a few days before. I don’t think I have ever met with anesthesia in my pre op appointment. Are you near a major medical center? Is there any way of doing this in-patient in a hospital? That *might* be a work around for you as an anesthesiologist might be more willing in a hospital setting. Ironically enough, if this tooth goes septic then she’ll get help. But you don’t want to let it get to that point, regardless of BMI. I’m sorry. I know this is no help, other than to know that this is not unique to Medicaid but regardless of insurance.

UMKC has a dental school that it does look like they have residency programs in oral surgery. Unfortunately, the link above does not, it looks like it only goes as far as dental education. For some reason, I can’t get into UMKC dental schools web page. None of the links provided work.

Surgeons often won’t do elective surgery on obese patients but require patient to lose weight before surgery. Variety of reasons poor healing, surgery is riskier etc Will the Medicaid dentist or Dental school refuse? Too complicated without general anesthesia? Deep sedation? Or does BT just yell and scream when they try? If she had 10 locals but still had pain, well maybe just go ahead and bear the pain. It’s transient . Surprised dental school doesn’t give iv sedation General anesthesia is a big step for removing a tooth Is tooth infected? If so she needs meds before removing tooth I had an impacted wisdom tooth removed by dentist breaking it into pieces then pulling the roots. No anesthesia or even local. Granted dentist was weird he kept trying to make me meditate but yeah painful but it got done br]Since BT is obese maybe the new glp meds such as ozempic can help . if she is diabetic or pre diabetic Medicaid will probably pay but not if only obese
Has she had a glucose blood test?

I'm dealing with Medicaid for DD now, all I can say is good luck. She is doing great with weight loss looks like it will not be paid for her so she is destined to be fat again. I don't think her heart can take it. She had an abnormal EKG with several things wrong. I'm going to appeal but I doubt it helps $1100 a month out of pocket for it? Right now taking it one day at a time. I've been told its hard to get here. But she would have been denied in Indiana too as in August they dropped her for getting too much SS. We are going to look into Canada if we can't get it or maybe Mexico though I'm afraid to go there anymore. We already pay for all our dental out of pocket. Our DD went to the school of dentistry in Houston to have a ton of molars take out of her small mouth and they did a great job.
I have the kids coming over for dinner tonight. I'm making baked ham, baked beans, deviled eggs, a vegetable plate, fried potatoes, and fried corn. And I baked a pineapple upside down cake last night. Things that son likes and doesn't get often. Making enough for them to take some home. Need to get at it. Oh, I'm also making Cole slaw because DD likes it. Hubs went to the store to get me rolls I can bake. I like Rhodes rolls, the frozen ones.

Our university has a great dental school, and that's where DS got work done, including surgical extractions in the past. The only thing is I don't know how long the wait would be, and it sounds like this young girl shouldn't wait much longer.

Got to get busy again. Hubs went to the store for me and picked up a few things. I have to bake the ham for 2 1/2 hours at 325. It's kind of dry I think I will put water in the pan to help steam it some.
Will do the baked beans and reheat them, need to get my deviled eggs done and chilled.
Need to vac, hubs and I picked stuff up. I don't have as much incentive to keep everything clean and neat unless someone is coming. Sad I know.
It's raining today and 51 out. After seeing what they had to do on our roof, I'm sure happy we had it done, I think a bit down the road we would have had leaking, so dodged that issue. Another lady here said they really had some damage. It's not good to wait so long to fix this stuff.

Would appreciate the help. I'm in St Louis. This is the original dental school that we went to multiple times. The last time I could hear teen and others screaming in pain from the waiting room. The dentist supervisor said I gave her 10 numbing shots and she just keeps feeling pain. I really can't take more rejection today. Haven't forced myself to call a few more places that's I've dug up but will power through after lunch. I'm trying to get through something resembling a workday while checking on teen every 30 mins bc she's in really bad shape today. Barely speaking to me but I managed to get her to move upstairs so I could monitor her safety. How far are you from Kansas City? UMKC has a dental school that it does look like they have residency programs in oral surgery. Unfortunately, the link above does not, it looks like it only goes as far as dental education. For some reason, I can’t get into UMKC dental schools web page. None of the links provided work. A solid 4 hours from KC. Six more rejections. Tagged in school district head social worker. Here's hoping she has more luck, resources and bandwidth.

I'm under contract on the car. I'll get it sometime next week, after my procedure.
It came in at a pretty good price point. They gave me too much for my trade in IMVHO. And I'm not telling them that. I think it is perfect for me. I am excited and thrilled and always the niggling in the back of my head. But, I can't stop life on a 30% chance of something happening.

Surgeons often won’t do elective surgery on obese patients but require patient to lose weight before surgery. Variety of reasons poor healing, surgery is riskier etc Will the Medicaid dentist or Dental school refuse? Too complicated without general anesthesia? Deep sedation? Or does BT just yell and scream when they try? If she had 10 locals but still had pain, well maybe just go ahead and bear the pain. It’s transient . Surprised dental school doesn’t give iv sedation General anesthesia is a big step for removing a tooth Is tooth infected? If so she needs meds before removing tooth I had an impacted wisdom tooth removed by dentist breaking it into pieces then pulling the roots. No anesthesia or even local. Granted dentist was weird he kept trying to make me meditate but yeah painful but it got done br]Since BT is obese maybe the new glp meds such as ozempic can help . if she is diabetic or pre diabetic Medicaid will probably pay but not if only obese Has she had a glucose blood test? Just going to say this reads like she's fat and poor so needs to tough it out. If you knew half of what this kid has been through, you'd be devastated. When she says she was in days of killing herself before she told me about the abuse, she's not kidding. She had no way out. Her physical pain tolerance is super high. She's only been clutching her jaw this last week. Tooth is infected and falling out in pieces week by week. I'm not brave enough to look in her mouth but DH did and gasped in horror. Ive seen the xrays. She did a course of antibiotics in April in prep for first attempt. No one will bother to prescribe them since. I ask each and every visit. She self soothes by overeating. That's the least of her problems. Add in antidepressants and something they have her on for bipolar without even a bipolar diagnosis and she's screwed weight wise. Psychiatrist went through 4 sleep meds before the current one which finally worked but happens to have a side effect of being an appetite stimulant. Kid needs to sleep. Psychiatrist will address the bipolar med once teen has two stable months. That can't happen until her tooth is treated and we can go back to ignoring bio mom and concentrating on normal life and healing. Then throw in her mom harping on her weight every fucking interaction and poor kid is doomed. Mom legit sat in hospital er with me watching seizures and tried to blame them on her weight. I had to had multiple doctors explain to teen that weight has nothing to do with her seizures they are ptsd and caused by trauma. Eta her therapist will address her eating habits when teen is stable. Also on the back burner are glasses. Teen completely failed, failed, failed school eye exam last year and bio ignored it. Teen suspects she has needed glasses for years and yet somehow she manages to excel at school. She knows it's her only ticket out of generational poverty and dysfunction.

Just spent an hour driving round trip to pick up teen's cousin. I am so proud of her for letting cousin and aunt know she's struggling. Cousin's visit is just the mood shift we both needed. Aunt said Azucena must really love you if she's willing to drive and pick up cousin and bring her home again later tonight. Yes, yes I do.
There's talk of them going to football game tonight at Cousin's high school. This means I have to figure out at least one person to teach seizure protocol and then I'll be completely on edge the two hours or so that she's gone. But she has to be a normal teen whenever possible.
