Well AFLAC apparently does not offer long term disability insurance unless it is through the person's place of work. Wife's place of work does not offer it and she will change jobs soon. The long term disability insurance is for my wife. So any other suggestions? thanks

Have you looked at Guardian/Berkshire disability insurance? It's been several years since I got my policy, but from the research I did at the time it was the best option for me. I haven't had to file a claim, so I can't attest to how much of a nightmare that process might be, but I've been satisfied in my other dealings with them.
There are some specific disability insurance forums you might want to Google and check out. There are numerous posts from people of all different needs/occupations, so you might get some ideas that relate to your situation
