Hello,
I will have a new situation with my taxes this year, concerning my HSA. Last year, my employer began a new program through our health insurance called Motion. By meeting certain "stepping/walking goals" each day, you could earn money from the insurance company (NOT the employer) that would be placed in your HSA quarterly. Last year, I earned $67 through this program.
How will this work with my taxes? I have not paid taxes on the $67, but they also did not come from me OR my employer. Will I receive some sort of form in the mail? I can't seem to find an example such as this on the IRS's website to tell me where to turn.
Any help would be most appreciated.

Without knowing all of the contractual details I can only offer an observation. It appears to me that the insurance company is offering you a discounted rate for your engaging in healthy activity that reduces their risk.

What's the description on the deposits in your HSA? Mine say employee or employer situation.
My company puts money in through completion of things on a 3rd party site and on my W2 only my contributions show up.

The deposit descriptions in the HSA account are "CURRENT YEAR EMPLOYER CONTR" for the money placed in my account from my employer (cafeteria funding), and "UHC MOTION REWAR UHC MOTION ######" (actual numbers are where the # signs are). I don't contribute any extra dollars to my account. (I know, I know. I should. But that's a discussion for another thread.)
I asked my HR rep at my company if the Motion dollars would show up on my W2 and she said that only any money from the cafeteria funding or my paycheck would show up there.
