DH received a check last year from an old job's 401K. He only had $471 in it so they sent him a check out of the blue. My understanding is that since the amount is less than $600 they don't have to send out a 1099. How do I enter it in a tax program? I am using Tax Act and they want the 1099 info which I don't have.

actually a 1099-R is required for payments that are $10.00 or more.... you should call the plan administrator..... From IRS.gov www.irs.gov/efile/article/0,,id=98114,00.html
