Some of my IRA accounts were registered as "rollover" from a 401K plan. Others were not even though they were also rolled over (from either 401K or Simple IRA).
I remember that at one time the forms (at Vanguard, etc.) did not used to indicate this selection but now they do.
Question is, "must" a rollover from a 401K be registered as a "rollover" IRA?
I believe the purpose of indicating this on the IRA registration is so that the funds could be rolled back into another 401K if this is ever desired. But are there any tax consequences if indicating "rollover" on the registration is not done?

No, I rolled mine into a rollover IRA once then combined it with my regular IRA to have fewer accounts.
