Yes, we do. DH's company pays part of the premium and we pay the other part. What percentage of the total each is, I don't know. I do know that our portion keeps going up and I assume the company's is too.

Yes, I have it. I pay part and my employer pays part. Before I had this job my previous employer provided benefits but the company they went with was horrible, so I dropped it and paid for health insurance with a different company on my own.

I have health insurance through the Province for the basics and then we have benefits at work for things not covered by OHIP. The big one in prescriptions. As the person that pays the benefits for the company it drives me wild. Benefits claimed is kept at 75% of the premiums paid. The company always pays more than folks are using. grrrrrr, I don't like the insurance industry.

Yes, Blue Cross/Blue Shield. Employer pays most, and I pay some. Still pretty expensive, about $400/month out of my pocket, and that's just for me. Copays are high for specialists, but drugs are very low as long as they are generic.

My wonderful DH is active duty military, so we both have health insurance through Tricare Prime Remote. He never has a copay for anything, ever. I have a prescription drug copay that runs from $3 to $27 per prescription, and a $100 copay of I go to the Emergency Room and am not admitted. There is no copay for doctor visits or labwork for me, as long as I go to a covered provider. We don't pay premiums for this health insurance.
My wonderful DH also has dental care through United Concordia (no premium, no copay), and so do I ... mine has a $12.86 per month premium, and I think it's a standard 70/30 covered services sort of plan.
While vision isn't covered by Tricare, because I'm diabetic I get one eye exam per year that is covered, so if I need a new RX for glasses, I get it then. (The glasses would not be covered.)

Yes, we only pay $2 per month for family coverage - company pays the rest ($826.29). I know the exact amount because I am in HR! We also have a prescription card and $30 co-pay (used to be $20). I am dreading 2014 when Obamacare kicks in.......we are screwed!

I am working as a temp so I have to pay 100% of my health insurance (purchased outside of my employer). I have a $5500 deductible for each of myself and my son so I basically have to pay everything out of pocket myself and would only use the insurance if one of us has a catastrophe. We had the COBRA supplement until the end of November but when it expired it was too expensive to continue to purchase insurance through my old company. I've been working as a temp for almost a year now but as long as I'm a temp, I'll have to buy my own insurance.

Yes, we both have it though our former employer (we're retired). DH pays $32/mo premium for a HDHP policy (a regular policy would have cost $132/mo) I pay $14 for a Medicare supplemental and $96.50 for Medicare part B premium. So we pay a total of $143/mo

Yes have health insurance through my work. Work pays the biggest portion and I pay the other which is taken out of my checks throughout the year. It is an HSA. Everything goes toward deductible including scrips and then when you reach the deductible everything is paid 100%. It has been good insurance. If you don't use all the money then it rolls over to the next year.

Health insurance is one of the biggest benefits I get from my late husband's company. It's fully paid for, i.e. no premiums, with only a $10 co-pay for my regular doc and the same for a specialist if my regular doc sends me to one. Prescriptions are also $10, unless they are generics then it goes down to $5. I've always opted-out of any insurance my employer offered me because it made no sense to pay for something that offered far less.
When I become eligible for Medicare I will lose it. So in a few years I will start shopping for a Medicare supplemental I can afford.

Yes, I have it and the larger part of the $500/mo premium is paid by my employer.
4 years ago, it paid out over $100K for 2 orthopedic surgeries and I expect that my surgery in a couple weeks is going to run around $60K.
I'm trying to stick it out here long enough to retire as my employer will pay a major percentage of Medigap insurance for both me and my spouse. Definitely a benefit that one shouldn't sneeze at.

Ours is paid for by DH's company... although, he's a part owner so technically, we pay for it. High deductible medical policy with an HSA; decent dental.

I have BC/BS and I pay 45% and the company pays 55% of the premiums. My share is about $218 per month. We have a deductible of a $1000 but the doctors visits are $30 ($50 for specialists). It isn't the best, but it certainly beats not having insurance or having to pay 100% of the premiums. We also have prescriptions that range from $30 to $50 but it seems that all my drugs are at the highest costs

Yes, I have insurance. Part paid by employer and part by me. I had BCBS for close to 20 years but had to switch 2 years ago because it got to be too much (the ex seems to think kids are free after they get out of HS). So since I have to pay all the insurances (life, medical, vehicle), medical, dental, vision, meds., etc. I had to find something more affordable. I did, and the coverage is similar. But I sure did like the security of BCBS which is taken pretty much everywhere.

My husband is retired from the USMC so we have Tricare standard. We have a $300 annual family deductible, and 20% co-insurance. We have the RX and they run 3-9 dollars depending on what I am getting. I make sure to budget my flex spending account each year to cover these items. So, I am very blessed!

I have employer sponsored BCBS. Employees pay about 20% for employee coverage but full price for other family coverage. Needless to say, I only cover me. DH has coverage at work but it isn't great. But my policy is a high deductible policy so I have to cover the first $2,500 of expenses before the insurance even kicks in but if I covered DH, we would have to cover the first $5K! And the premiums would eat our budget. We are both diabetic so I hit my deductible pretty quickly and I do have an HSA that just almost covers the deductible. It would have covered it if the deductble hadn't gone up this year.

I have an individual plan with a $2K deductible but they do waive co-pays for preventative care at least. I pay about $170/mo and also contribute to an HSA each month to help offset the deductible should I ever need it. I used to have a really horrible plan through work that I paid for 100% that was about $110/mo but it covered absolutely nothing so I felt like I was lighting the money on fire. I just had to wait though for a few things to "expire" so I could actually get an individual policy. I'm hoping after baby #2 that I can lose about 30 lbs and get a better rate but even at $170/mo I'm ok with it.

I am currently on COBRA. In 2010, when I was working I paid $110/month for a modified HMO type plan (had deductables & 20% co-pays). When I had to COBRA it went to $383,
This year it went up to $438. I suspect this is because they can no longer charge the deductable & co-pays on many preventative care items (annual Dr visit, Pap, Mamo, routine blood workup......).

High deductible catastrophic insurance but it's reasonable compared to BC/BS for us. We are very careful to keep ourselves in good physical condition and eliminate sports like downhill skiing that have high risk. Is this an informal random survery for a college term paper?

I have insurance through my employer. United Healthcare. $400 deductible (it's just me) with 20% copay for in network and 30% for out of network, after deductible. $20 taken out of each paycheck. The only year I ever actually reached my deductible was when I had to go to the ER for kidney stones. I had been sick a few times earlier that year, plus had my annual. So I was close to my deductible when I landed in the ER and the visit was about $130.
Prescriptions are included in that. $10 copay, though it's actually cheaper for me to go with the $5.40 generic at my Target pharmacy for my blood pressure meds.
Also includes vision. $10 for an eye exam. Covers 70% of glasses but nothing on soft contacts, which is what I wear. So I pay OOP for those.
Dental is $5 from every paycheck. Two free cleanings and checkups per year, even if you go out of network. I don't know deductible and copays without looking those up as I've never had to have any dental work done. That's $260/year for dental. It would actually be cheaper for me to just pay OOP for checkups every year, as my dentist only charges $65 per visit, but I want to have the insurance anyway, because, at 28, my good fortune will someday run out and I'll be looking at some kind of dental work. I figure I probably can't go much longer without getting at least a cavity.

We ALWAYS have health insurance. I don't care if I have to cut out every luxury in my life, I will have health insurance! We currently get it through DH's work, and it is relatively inexpensive for us. his part is $26/month, with me added its another $200/month, but it is great insurance and I am thankful to have it. When I go full time in May my insurance will also be $26/month In the past, when we were both part-time workers and full time college students, we bought individual plans through Humana. It was like $75/month each, so not bad, but it didn't cover maternity (thankfully that was never an issue at that time). I have an under active thyroid that requires monitoring through blood work 3 times a year and medication every month, so going without insurance would not be wise.

yup, health and dental for just me. I pay some out of my paycheck, not sure how much (will be checking my paystub when I get back to the office to verify 2011 deductions. I have direct deposit).
health is BCBS, $15/20 co-pays for office/specialists, $100 for ER. however, I was able to have an MRI on a Saturday, one day after it was ordered by my primary this past summer. there was no co-pay for it, either. talking to other hockey players with similar injuries, some had to wait weeks for their insurance to get off their duffs and approve one.
dental is Delta. no co-pay for cleanings, $2k lifetime coverage for orthodontics (which I've already used as an Invisalign patient), and I think it cost me $150 to have all 4 wisdom teeth extracted almost 3 years ago.
