Right now, I'm sitting on a survey that was sent to me from the hospital that is did my gall bladder surgery a couple months ago. I threw the first one out without opening it, so they sent me a second (isn't that nice of them?).
The problem is, that hospital stay was a clusterfu** of magnificent proportions. Some nurses were good, others were hideous. There was absolutely NO communication between ANYONE and I remained NPO (no food or water) for days because they *may* do a procedure (that wasn't even on the schedule). But no one knew anything and I was just left there hanging.
This was made worse by the fact that my hip replacement a few weeks earlier, my care was superlative, so I have a recent basis for comparison.
I hate doing surveys to begin with, they never ask me the questions I want to answer and the questions never have the answer I want to give. Now, it seems like everytime I get some sort of service, they expect you to fill out a survey.

Tell them what you said above. Include an extra paragraph if needed. Someone in admin should know it sucked.

Just attach a letter saying the above with more details as needed.

Copy and paste that into word, add any extra info you feel is applicable. Print it out and write on the survey - "see enclosure", and mail it all back.
