I recently purchased 20 yr term life insurance of $750 K for $45.93 a month (I am 40-ish). They offered an option that paid the monthly insurance for you if you become disabled -the cost was an extra $9.37 added to your insurance. Would that have been worthwhile to purchase as well?

Probably not, unless you don't have any other disability coverage and would be worried about paying your life insurance premium should you become disabled. Probably more cost effective to have enough disability coverage from work and an individual policy that would pay all your essential bills, including the life insurance. $9 to cover $45 seems expensive.
