My daughter received a 1099-Q from the state tuition program for 2011. I don't recall her ever receiving this before & this is her 5th year of college (post-bac program now).
She does not have earnings, and is my dependent. Do I need to file this 1099-Q distribution on my taxes somewhere? She was and is still in school, so all the funds were used for qualified education expenses.

You should have received a 1099-Q for each year you had withdrawals from your qualified tuition plan. That being said, if no portion of the distribution is taxable it is not reported on your 1040.
Be sure to reduce any qualified educational expenses by any tax-free educational assistance (scholarships and grants, but not loans) when determining the total qualified expenses.
