I dunno about other places, but the eye test where I live is a joke. It's just stick your head in this thing that's essentially a large viewmaster and read a row of letters. I passed it with flying colors last time because I'm near sighted so letters that are 6 inches from my face are perfect. Meanwhile I now pretty much wear glasses when I watch TV because it's blurry as hell. One of my eye doctors a few years ago more or less said I should be wearing glasses when I drive. Still doesn't say it on my license, but I drive fine without glasses as long as I don't need to read street signs until I'm like 5 feet away lol.
ETA: I do drive with glasses 99.9% of the time because I don't like driving when things are blurry even though it's not so blurry that I can't see the car in front of me and realize how far away it is - I just can't make out their license plate. But I have a few times unexpectedly - ie staying some place later than I thought and I only brought my rx sunglasses with me or being out and a storm coming in making it too dark to use my sunglasses.

BIL found out the driver's license office was of no use in taking away his mother's driver's license.
Her doctor had a heart to heart with her. She refused to take his advice and renewed anyway.

My great aunt was blind in one eye and legally blind in the other and didn't stop driving till a couple years ago. Then my grandma had a friend just as bad that never lost her license despite failing the eye exam MULTIPLE times. Plus in Iowa you only renew every five year so if you passed the eye exam then you were good to go for quite awhile. My mom finally decided if her and my grandma wanted to go out Thelma and Louise style so be it, hopefully they wouldn't take anyone else with them.

My ex husband's grandfather was still driving short distances in his 80s. Everyone thought it was fine then he got into an accident. He lived, but his girlfriend that was in his car died as well as the passenger in the other car. He was absolutely devastated by this. Yes they took his license away then, but it was too late. 2 people dead.
