Most influenza outbreaks disproportionately kill juvenile, elderly, or already weakened patients; in contrast the 1918 pandemic killed predominantly previously healthy young adults. Modern research, using virus taken from the bodies of frozen victims, has concluded that the virus kills through a cytokine storm (overreaction of the body's immune system). The strong immune reactions of young adults ravaged the body, whereas the weaker immune systems of children and middle-aged adults resulted in fewer deaths among those groups.[11]
Historical and epidemiological data are inadequate to identify the pandemic's geographic origin.[2] It was implicated in the outbreak of encephalitis lethargica in the 1920s.[12]

I guess we will see where we go from here then... My parents just got home and my mom said some people were coughing on the plane... She has been sick since Sun... We are still doing well here, lots of cleaning that's for sure. Thanks for the heads up.. Great call on NVAX as well!! 6 handle now! You may be closer to retirement than you think..

Commentary US P&I Death Rate Crosses Epidemic Threshold Recombinomics Commentary 22:30 January 16, 2014
Tomorrow’s CDC Week 2 FluView will show a jump in the P&I Death rate to 7.5% (from 6.9%), which will be above the epidemic threshold. In addition, pediatric deaths will double from the 10 reported through week 1 to 20 for the total reported in week 2 (MA, KY, LA. OK, TN 3, TX 3). These increases reflect the severe and fatal lab confirmed cases reported largely in the south in recent weeks.
Although relatively low levels were still reported in many northern states in the week 1 report, the numbers are now beginning to increase in the lagging northern states. Both P&I and pediatric deaths are lagging indicators, and both numbers are expected to rise significantly in upcoming weeks.
Earlier indicators, such as the severe and fatal H1N1 cases reported in Dallas County or California will be reflected throughout the country because of the increase in polymorphisms that target human lungs. L194I, D225G, and Q226R were noted in initial sequences released by the CDC (largely in egg isolates), and the evidence for a higher level of these changes was supported by the recently released sequences from China, where most of the sequences were from egg isolates and the vast majority of these isolates had at least one of the changes that target huan lung.
Below is a list of the US cities with at least 5 P&I deaths reported in week 2, as well as a rate of at least 10% in the week 2 report.

Commentary H1N1 Tamiflu Resistance Spreading In Mississippi Recombinomics Commentary 16:00 January 17, 2014
The CDC has released another series of H1N1 sequences which included two more sequences (A/Mississippi/28/2013 and A/Mississippi/29/2013) from Mississippi with H274Y, signaling clonal expansion of Tamiflu resistance in Mississippi. The gap in collections dates (December 7 and November 18, respectively) coupled with the November 4 collection date for A/Mississippi/11/2013, strongly suggests that H274Y is transmitting in Mississippi, as well as Louisiana, and is more common than indicated in CDC weekly updates in FluView.
The recent acknowledgement of H274Y in Texas suggests it is widespread in southern states and is widely transmitting (also California?). This transmission of H274Y is also supported by the sequences in Sapporo, Japan, which was supported by the recent Eurosurveillance paper, as well as recent sequences from China, which suggests H274Y has been transmitting in China since September.
These clusters and examples of transmission via clonal expansion, raise serious surveillance and reporting of resistance by the CDC.

Commentary Alberta ex-Beijing H5N1 Has RBD Change D225R Recombinomics Commentary 02:30 January 25, 2014
The Health Protection Agency of Canada (HPAC) has released (at GISAID) a full set of sequences, A/Alberta/01/2014, from the nurse who developed symptoms while returning to Canada from a trip to Beijing. The HPAC is to be acknowledged for the rapid release of these important sequences. The isolate was a recombinant with 7 of the 8 gene segments related to clade 2.3.2 waterfowl sequences in Vietnam, as well as wild birds isolates from a broad geographic range in eastern Asia. The PB2 however is closely related to H9N2 poultry sequences circulating in northern China.
Moreover, the H5 sequence is also related to for isolates from northern China, a tiger isolate from Jiangsu, A/tiger/Jiangsu/01/2013, as well as three environmental sequences from sewage in Huzhou (A/Environment/Huzhou/C291-6/2013, A/Environment/Huzhou/C291-7/2013, and A/Environment/Huzhou/C291-10/2013).
The H5 sequence also had two receptor binding domain changes, R193K and D225R. The D225R change is a particular interest, because most H5N1 sequences have D225G, which targets the lungs, but the fatal infection in the Alberta nurse presented as encephalitis and without a cough. The sequence from Canada was from a cloned isolate, which was presumably from mammalian cells, which would have selected against D225G. However, D225R has been reported in H5N1 bird flu from a duck in Hunan, A/duck/Hunan/15/2004, and may be responsible for neurological targeting instead of lower respiratory tract.

Today’s CDC week 4 FluView will show a jump in the P&I Death rate to 8.8% (from 8.1% in week 3). This rise ensures a second severe flu season in a row in the United States. Last season (2012/2013) generated a P&I peak of 9.8%, which was largely due to the dominance of H3N2, which historically takes a toll on the elderly, who usually account for 95% of the flu deaths.
However, this season pH1N1 dominates, and since its emergence in 2009, the number of deaths has been blunted by cross protection of seasonal H1N1 in older patients. Consequently, the P&I death arte was unexpectedly low in the 2009/2010 season when pH1N1 crowded out competing serotypes, leading to a low death rate in the elderly.
In contrast to to 2009 however, this season the number of deaths has been unexpectedly high since most elderly still have cross protection and younger targets would have immunity due to natural infections by pH1N1 as well as vaccination formulas which have had a pH1N1 target for the past five years.
The domination of pH1N1 this season suggests it has evolved away from the natural immunity or immunity due to vaccinations, which may have been impacted by use of the same pH1N1 target (A/California/07/2009). The high P&I rate this season raises serious questions about the methods and assumption used in the selection of vaccine targets.

The above translations from the Guangdong Province Health and Family Planning website describe two recent H7N9 bird flu cases (28M & 5M) from Huai town in Zhaoqing Huaiji county. These two cases were reported one day apart and the confirmation of a child with mild or stable H7N9 that is geographically and temporally linked to a severe or fatal adult case raises concerns that the young cases are contacts of the severe cases.
China has focused surveillance on severe hospitalized cases, but recently has cited milder cases, with these linkages in time and space. Sequences from these clusters have been largely lacking, but these associations are far from random.
The above cluster in Guangdong Province is within 100 miles of a similar cluster in Hunan Province (38M and 8F) and each cluster is less than 50 miles from the first H7N9 case (56F) in Guangxi Province (see map).
These cases and clusters raise concerns that the efficiency of human to human (H2H) H7N9 transmission is increasing.
Release of sequences from these clusters, as well as the relationships between the cluster members, would be useful.

A 28-year-old man from Zhaoqing City, Guangdong Province, who became ill on 24 January 2014 and admitted to hospital on 26 January. He is currently in a critical condition. The patient has a history of exposure to a live poultry market.
A 5-year old boy from Zhaoqing City, Guangdong Province, who became ill on 29 January and admitted to hospital on the same day. He is in a stable condition. The patient has a history of exposure to a live poultry market.
The above comments from the February 3 WHO update provide additional detail on the H7N9 bird flu cluster in Huai town in Huaiji in Zhaoqing City in Guangdong Province (see map). The index case (28M) developed symptoms on January 24 and was hospitalized on January 26. Three days later the child (5M, who is likely a contact/son of the index case) developed symptoms and was hospitalized the same day, January 29, strongly suggesting the hospitalization was linked to existence of a relationship between the two Huai residents. This pattern of a mild / stable child being confirmed a day after a severe adult case from an identical location supports a series of clusters associated with these milder / younger cases, as noted for clusters in Jiangyong, Hunan and Sanjiaozan, Guangdong.
However, the relationships between the confirmed cases define these clusters, and sequences have not been released.
This lack of transparency raises serious pandemic concerns.

The above translations from the Guangdong Ministry of Health January 11 and January 18 reports on two cases (29M and 5F, respectively) from the Baiyun District of Guangzhou City (see map) describe patterns seen in more recent clusters. The confirmation of a critically ill adult H7N9 bird flu case is followed by a mild case involving a child who is from the same location as the adult. Unlike more recent updates, the disease onset dates for the two cases are given in the local agency rport, fully supporting human to human (H2H) transmission. Moreover, the development of symptoms by the child after the adult is H7N9 confirmed leads to rapid hospitalization and treatment which produce a mild or stable clinical presentation of the case.
Although the January 18 announcement of H7N9 confirmation in the child fails to note the relationship between the two cases, multiple subsequent Chinese media reports note that the 5F is the daughter of the 29M index case.
However, none of the recent agency reports on Guangdong (Huai and Sanjiaozhen) or Hunan (Jianygang) clusters have noted that the cluster members were related or were contacts, althoiugh the February 3 WHO update does note the child (5M) in the Huai cluster was hospitalized the day he became symptomatic.
The acknowledgement that the above cluster was familial involving a father and daughter, increases the likelihood that each of the three recent clusters in Guangdong and Fujian Provinces are also familial, although the Ministries of Health in China, as well as WHO, fail to note the relationship of the cluster members, raising serious pandemic concerns.

The above comments from the January 14 and 20 WHO H7N9 bird flu updates confirm the 11 day gap in disease onset dates between the father (29M) and daughter (5F) cases in the familial cluster in the Baijun District of Guangzhou City, (see map) as seen the Guangdong Province Ministry of Health reports. The WHO report also notes that the daughter was hospitalized one day after disease onset, which was associated with a mild clinical presentation and antiviral treatment led to a subsequent discharge.
Although the father / daughter relationship was widely reported in Chinese media prior to the January 20 WHO update, the update failed to note the relationship. Consequently, the familial cluster was largely not reported in English language summaries by CIDRAP and ProMED, which rely on blogger reports to keep tallies of confirmed cases. This absence of cluster reporting is also true for the three recent clusters in Guangdong (Huai and Sanjiaozhen) and Hunan (Jiangyong), which were confirmed in the past few days.
The failure of the health agencies in China as well as WHO to report these clusters is remarkable. WHO notes that there is no sustained transmission, although China does not include mild cases in their surveillance unless the cases are contacts of H7N9 confirmed cases, and WHO fails to provide the most basic information, such as the relationship between conformed cases.
China undoubtedly knows that the mild cases are largely contacts, because China doesn’t routinely test mild cases that are not contacts. Similarly, the relationship between confirmed cases is undoubtable included in IHR reports, but WHO withholds that information from the public updates.
The lack of transparency on clusters, coupled with the dearth of sequences from these clusters seriously increases pandemic concerns.
This obfuscation on the clusters and sequences remains hazardous to the world's health.

The above translation from the February 4 update from the Guangdong Department of Health indicates that Huai H7N9 bird flu cluster has grown to three with the addition of another child, 4F. Although the above description does not cite the town, the above case involving another child that is mild or stable and in the same location as a recent confirmed adult (29M) strongly suggests that the above 4F will be linked to the current cluster in Huai town in Huaiji County in Zhaoging City.
The growing cluster, combined with the withholding of sequences and the relationships between cluster members, continues to raise pandemic concerns.

The confirmation of H7N9 in a severe adult leads to monitoring of contacts. The hospitalization of the older child one day after disease onset, and the “stable” condition strongly suggests that early hospitalization and treatment was due to the relationship between the children and the H7N9 confirmed adult, who was also from Huai town in Huaiji in Zhaoqing City in Guangdong Province (see map).
More information on the relationship between the severe case (28M) and the two children (5M & 4F), as well as the relationship between his home and that of the children, would be useful. Release of sequences from all three H7N9 confirmed Huai cases would also be useful.

The above comments from the February 3 and February 4 WHO H7N9 bird flu update describe a cluster that matches the pattern seen in human to human transmission among contacts, who typically are family members (and for the recent cases the family members are children).
The adult index case is diagnosed as critical or severe (if not dead), with a gap between disease onset and hospital admission. Severe cases with poultry exposure are the most common cases tested. For the above case there was a 6 day gap between disease onset and hospitalization.
Contacts develop symptoms well after the index case because transmission happens when the viral load in the index case increases. Contacts of confirmed or suspect cases are monitored and hospitalized 0-1 days after disease onset. These cases are typically described as mild or stable, and recover due to early hospitalization and treatment. The above case was hospitalized a day after disease onset. Although the WHO update cites a serious condition, the initial report on the case was stable.
Both of these cases were from Jiangyong County in Yongzhou City, (see map) strongly suggesting they are linked and will be cited as a familial cluster, as has happened for the Guangdong clusters in Baijun (29M father and 5F daughter), Sanjiaozhen (37M father and 2F daughter) and Huai (5M & 4F cousins living together, relationship to 28M unknown)

Jeddah MERS Export To Greece Raises Concerns Recombinomics Commentary 17:30 April 20, 2014
The patient is a 69-year old male Greek citizen residing in Jeddah, Saudi Arabia, who travelled back to Greece on 17 April. While in Jeddah, the patient consulted a hospital on 8 and 10 April for a febrile illness with diarrhoea, and was given a probable diagnosis of typhoid fever. Of note, he regularly visited his wife who was hospitalised from 31 March to 5 April in the same hospital for a confirmed typhoid fever.
The patient sought medical care upon arrival in Greece on 17 April. The evaluation revealed a bilateral pneumonia and the diagnosis of MERS-CoV infection was made at the National Reference Laboratory for Influenza.
The above comments from the April 20 WHO update provide additional detail on the MERS export (69M) from Jeddah to Greece. The wife of the case was diagnosed as “confirmed typhoid fever”, which was also said to be probable for the latest case. However, testing in Greece identified MERS.
In the same time frame Jeddah exported MERS to Jordan and Malaysia, signaling widespread MERS in Jeddah. The KSA-MoH has reported 46 confirmed cases in Jeddah in 9 daily reports between April 10 -19, with incremental increases from 3 cases per day for April 10 & 11 to 6 or 7 cases per day for the past 4 days (3, 3, 4, -, 5, 5, 6, 7, 6, 7).
The failure of KSA to diagnose MERS in this case raises concerns that reports indicating cases were far higher than reported due to physicians reporting cases orally and not in charts, which have misdiagnoses to explain symptoms as well as fatal outcomes.
The absence of WHO reports on these Jeddah cases (the most recent WHO report on the above cases, April 14, cites the age and gender for the three cases reported by the KSA-MoH on April 10, but has no information on dates for disease onset or hospitalization, or any information the patient’s condition) increases concerns.
The delays in reporting the most recent 43 Jeddah cases also raises concerns that the KSA-MoH reports, which have no dates, are not reporting cases in a timely manner, are not IHR compliant.

Novel MERS Jeddah Sub-Clade Defined - Includes Q833RRecombinomics Commentary 19:00 April 30, 2014

The above comments by Christian Droston, describe three nearly complete MERS sequences from three Jeddah (Jeddah_C7569, Jeddah_C7149, Jeddah_C7770) cases which were included in the phylogenetic tree published at the Virology Institute website, which also had the sequences for these three cases. The tree indicated the sequences were novel and formed a sub-clade devoid of prior MERS sequences.
An anysis of the three sequence identified 12 polymorphisms (listed below) which were present in all three sequences from the Jeddah cases, and were not present in any public MERS sequence. Thus, the 12 markers define the novel MERS sub-clade and raise concerns that one or more of these changes is responsible for the dramatic spike in MERS cases, which have led to at least five exported cases from Jeddah ( to Jordan twice, Malaysia, Greece, and UAE).
None of the 12 changes are non-synonymous in the spike gene (positions 21456-25517), CORRECTION: A23953G is non-synonymous and encodes Q833R but these changes clearly identify the novel sub-clade. Therefore, short sequences (such as positions 28778-28941) should be published to characterize the 25 viruses from Jeddah cases which are cited above. These cases have identical spike protein sequences, which also match sequences not from the Jeddah sub-clade, although a spike gene sequence from positions 23804-23953 would include two of these unique polymorphisms, including Q833R.
Several of these 12 polymorphisms appear to be present in a camel sequence that was published in a phylogenetic tree published by Andrew Rambaut, based on the presence of Qatar_camel_2 (sequence by Bart Haagmans) on the branch with the 3 human sequences. The sequences are described in a submitted paper to EID (Andrew Rambaut, personal communication).
It would be useful if the authors published camel_2 and related sequences on their website, as was done for the human sequences cited above, as well as websites of sequencers generating SARS sequences in 2003.
The role and distribution of this novel sub-clade appears to be linked to the explosion of cases in Jeddah, and the similar sequence in a camel in Qatar may be related to the MERS super-spreader (45M) in Abu Dhabi, since his fatal infection may be linked to an earlier fatal case (68M,) at the same hospital, who raised camels. 12 Polymorphisms Unique to the Three Jeddah Cases

WHO - Cause of Jeddah MERS Primary Case Spike Unknown Recombinomics Commentary 16:00 May 7, 2014
The reasons for the increase in the number of primary community cases, as well as the infection route, remain unknown.
The above comments from the WHO EMRO investigation of MERS cases in Jeddah continue to raise concerns that a novel sub-clade has emerged in Jeddah. The WHO report did note spread in hospitals and the need for more infection control, but the cases in western KSA have not been limited to Jeddah. The most recent MoH MERS update has cited cases in Jeddah, Mecca, Medina, and Taif.
The spike in Jeddah cases beginning in mid-March has raised concerns about a novel sub-clade. Sequences from three cases in two hospitals collected in early April (Jeddah_C7569, Jeddah_C7149, Jeddah_C7770). has increased concerns because all three sequences are virtually identical and include 11 polymorphisms not reported in any prior human cases. These sequences were released at the Drosten website and 25 partial sequences from the spike gene from additional Jeddah cases were said to be identical. However, these sequences, or location within the spike gene, have not been made public. Similarly, recent remarks have cited sequences from two more recent cases in Jeddah and one case in Mecca. All six sequences were said to “look completely normal” raising concerns that all six sequences formed the same novel sub-clade containing all or most of the 11 polymorphisms that defined the sub-clade.
If sequences match, concerns would be increased with regard to a novel sub-clade similar to the emergence of a novel SARS-CoV sub-clade in Hong Kong in 2003, which then spread to Singapore, Hanoi, and Toronto lead to significant nosocomial outbreaks.
The sequences from the three recent cases have been withheld. E-mails to the Drosten lab today and yesterday have not been answered, which is also true for e-mails to Andrew Rambaut on May 5, 6, and 7. The sequences or a phylogenetic tree would address the relationship between the first three cases, and the three more recent cases. These sequences should be released immediately.

Withheld Jeddah & Mecca MERS Sequences Raise Concerns Recombinomics Commentary 19:30 May 7, 2014
As long as the sequences are in the draft stage, we are making them available on our homepage (http://www.virology-bonn.de) and provide them to the epidemic blog (http://epidemic.bio.ed.ac.uk).
The above comments from a letter by Christian Drosten posted April 26 on ProMed suggested that Jeddah MERS sequences would be made available almost in real time, as happened during the 2003 SARS-CoV outbreak when sequencing labs posted results on their website. In 2003 labs in China, Hong Kong, Singapore, Canada, and Taiwan released such sequences almost in real time, and the above comments from the Drosten lab suggested sequences would be quickly posted. This pledge was important because virtually all samples from Jeddah were being sent to his lab, and it was unclear if exported cases from Jeddah to Jordan, Malaysia, Greece, United Arab Emirates, or Egypt would lead to rapid release of the exported sequences.
Shortly after the announcement was made on ProMED, the Drosten lab released three nearly complete sequences from collections in early April at his website (Jeddah_C7569, Jeddah_C7149, Jeddah_C7770), along with a phylogenetic tree that showed that all three Jeddah sequences were virtually identical. Similarly, the Rambaut lab posted a phylogenetic tree which also showed the three sequences were closely related to each other, as well as a camel sequence from Qatar (camel_2). When Recombinomics notified the Rambaut lab that a commentary was being prepared on the results, Andrew Rambaut asked that his tree not be used because the camel_2 sequence had not been made public. He also noted that he was pulling down the tree and asked that Recombinomics post a note that the tree had been made private, which was done.
The initial Recombinomics commentary used the tree posted on the Drosten website, which also showed that the three Jeddah sequences were virtually identical, raising concerns that the sequences from the Jeddah cases represented a novel sub-clade that contained 11 polymorphisms which were in all three human sequences and not reported in any prior human cases.
Another paper was subsequently published with camel sequences from Taif, KSA. Although the accession numbers were given, these sequences could not be accessed at Genbank until this week, but the Rambaut lab was able to put up a new tree with the Taif camel sequences and one of the three human sequences from Jeddah. It is unclear if this representation confused the CDC, because in their telebriefing on the MERS case in Munster, Indiana they noted that there was one sequence from Jeddah cases, when in fact there were three. The CDC cited concerns of an emerging sub-clade, but with only one sequence conclusions couldn’t be drawn.
However, there were three nearly complete sequences, as well as 25 partial spike gene sequences from additional cases in Jeddah, all of which were said to be identical, raising additional concerns that the Jeddah cases were due to the emergence of a novel sub-clade, as happened in 2003 during the SARS CoV outbreak. However, the 25 spike gene sequences were not posted at the Drosten website and the spike positions used in the sequences were not made public. In fact there have been no updates at the Drosten website since the first three sequences were released and the Rambaut website has a phylogenetic tree with only one of the Jeddah sequences.
Recently, an interview of Drosten cited three additional sequences from more recent cases in Jeddah and Mecca, which were said to be “completely normal” which was also used to characterize thre first three Jeddah sequences, raising concerns that the three recent sequences were closely related to the first three sequencing signaling clonal expansion in Jeddah and spread to Mecca.
Attempts to clarify these relationships have failed. A e-mail to the Rambaut lab on May 5 asking about the absence of any new sequences was not answered. Similarly, e-mails to the Drosten and Rambaut labs yesterday and today seeking clarification on the three new sequences also was not answered by either lab.
Publication of the three recent sequences or a phylogenetic tree with the new sequence at the Drosten or Rambaut websites would address the clonal expansion question. The withholding of this information highlights the dangers of allowing one lab to all of the Jeddah sequencing.
These sequences should be released immediately.
The withholding of this information is hazardous to the world’s health.

Jeddah MERS Contact Testing Recombinomics Commentary 03:15 May 8, 2014
C.D.: You cannot compare the new numbers to those from a few months ago. Until the 26th of March, 459 tests had been done in all of Saudi Arabia this year. Then in just 1 month, just in the city of Jeddah, 4629 PCR tests were done. Something dramatic changed, and that is the case definition.
Before, tests were done on patients who had pneumonia and required [intensive care]. But now people are being tested not because they are sick, but because they had contact with a patient. Some of these tested positive, but many of them don’t really get sick.
The Saudi Arabian Ministry of Health routinely screens all close contacts of patients in whom MERS-CoV infection has been diagnosed, and more than 3000 people have been screened to date. We recently identified seven health care workers with MERS-CoV infection (two of whom were asymptomatic and five of whom had mild upper respiratory tract symptoms) through screening of single sample nasopharyngeal swabs by means of a real-time reverse-transcriptase–polymerase-chain-reaction (RT-PCR) amplification test, with amplification targeting both the upstream E protein gene (upE) and open reading frame 1a (ORF1a) for confirmation. The above comments (in red) are from the Science Insider story “MERS: A Virologist's View From Saudi Arabia” which claims that the recent spike in Jeddah MERS PCR confirmed cases was due to a March, 2014 change in the case definition, leading to increased testing.
However, the comments (in blue) from a NEJM paper entitled “Middle East Respiratory Syndrome Coronavirus Infections in Health Care Workers” which was published in August of 2013, cited more than 3000 tests on contacts on MERS confirmed cases. This testing identified seven health care workers who either had mild symptoms or were asymptomatic, confirming that there was no change in the case definition in KSA in 2014.
It is well known that there has been an increase in beta coronavirus cases in the spring. In 2003 there was an explosion of SARS-CoV cases and international spread almost exactly 11 years prior to the explosion of MERS-CoV cases in Jeddah. Similarly, the nosocomial outbreak in the ICU in Zarka, Jordan was in the spring of 2012, while a larger nosocomial outbreak in Al Hasa in eastern KSA was reported in the spring of 2013.
Thus, the increase in MERS cases in Jeddah hospitals the spring of 2014 lead to increased testing of contacts, which was due to more PCR confirmed cases, not a change in the case definition.
The claim that the testing caused the spike in cases because the case definition changed is clearly false and Science should retract the statement.
Moreover, the misrepresentation of the Jeddah case spike also raises concerns regarding the comment that the recent MERS sequences from Jeddah and Mecca, like the first three sequences released from early April Jeddah cases (Jeddah_C7569, Jeddah_C7149, Jeddah_C7770), "all look completely normal." This statement should be supported by release of the recent sequences at the Drosten website, as pledged in the ProMED letter published on April 26.
The failure to release the sequences, or answer the most basic questions regarding the relationship between the first three sequences and the subsequent three sequences, strongly suggests that all or most of these sequences form the same novel sub-clade with unique polymorphisms, as noted.
The Drosten lab has exclusive access to these critical samples, and sequences should be made public immediately.

Jeddah and Mecca MERS Sequences Match Recombinomics Commentary 12:15 May 8, 2014
C.D.: There has been a lot of discussion about whether the virus has mutated to pass more easily from human to human. But we have sequenced three genomes from samples taken early in April in Jeddah, where many of the cases occurred, two more from later in the Jeddah outbreak, and another one from a patient in Mecca. They all look completely normal.
The above comments raised concerns that all six sequences from western Kingdom of Saudi Arabia (KSA) represented clonal expansion of a novel sub-clade. Chistian Drosten has confirmed via e-mail to Recombinomics that all six sequences were closely related, confirming clonal expansion.
A similar clonal expansion was seen in 2003 for the SARS-CoV at the Metropole Hotel in Hong Kong. The sequence contained a 29 BP deletion, which was subsequently found in almost all SARS-CoV sequences from subsequent cases, including those in Singapore, Hanoi, and Toronto.
In the recent CDC telebriefing introductory remarks noted the explosive spread of SARS-CoV due to a novel sub-clade, but noted that for Jeddah only one sequence was available even though three sequences (C7149 and C7770 from hospital A on April 3 and 7, respectively as well as C7569 from hospital B on April 5) had been made public at Christian Drosten’s website on April 26, well in advance of the CDC telebriefing on May 2.
Initial comments in ProMED indirectly cited infection control issues in Jeddah, but the first three sequences were from patients in two different hospitals, and the presence of the same sub-clade in two later cases, as well as a case in Mecca, supported clonal expansion.
There have been six confirmed exports of MERS from Jeddah to five different countries (Jordan, Malaysia, Greece, United Arab Emirates, and Egypt), but onward transmission has only been cited in Amman, Jordan, where the index case (25M) infected a paramedic (28M) and his brother (28M), although media reports cited additional symptomatic contacts in Quarayat, KSA.
Christian Drosten is to be commended for the early release of the first three Jeddah sequences and the confirmation that the subsequent three sequences match.
Posting of the three more recent sequences may help convince other MERS sequencers to follow suit and promptly release sequences, which would be useful.

Christian Drosten at the Virology Institute in Bonn has uploaded the 3 recent MERS sequences from patients in Jeddah (C8826 and C9055) collected on April 12 and April 14 from hospital A and C, respectively, as well as Mecca (C9355) collected on April 15. The three earlier sequences were from two hospitals ((C7149 and C7770 from hospital A collected on April 3 and 7, respectively as well as C7569 from hospital B collected on April 5)). The release of these nearly full sequences from patients in Jeddah and Mecca are to be commended and will hopefully encourage other sequencing labs to make sequences available on lab websites as soon as possible.
The first three sequences had nine polymorphisms that were in all three Jeddah patients, but not in any published sequence. Subsequently, sequences from camels in Taif were described in an mBio paper and four were closely related (see list below). However, none of the camel sequences had all 9 polymorphisms. Two camel sequences (378 and 505) had 5 of the 9 polymorphisms and none of the camel sequences had A23953G, which encoded Q833R in the spike protein, or T28778A, which encoded L6Q in ORF8b.
The three newly released human sequences had all none of the polymorphisms. Thus, all six humans sequences from three hospitals in Jeddah as well as a hospital in Mecca uniquely matched each other signaling clonal expansion in western Saudi Arabia.

He came to Indiana for a conference and when he became ill he went to stay with family according to the Indiana State Department of Health.
The above comments indicate the MERS infected Health Care Worker (HCW) flew to Chicago to attend a meeting when he arrive at O’Hare at 10:05 AM on April 24. He would have attended the meeting and then would have taken a bus to relatives in or around Munster, Indiana.
The meeting that would fit into the above timeline was the 10th Annual Meeting of American Physician Scientists Association (APSA) which was held at the Fairmont Hotel between April 25-27 in the Loop (downtown Chicago).
The attendance of a conference was not cited in the CDC telebriefing, but the comment on the patient arriving in the United States on April 27 was confusing since the arrival time of the flight from Riyadh to London (British Airways Flight 262) and the departure time of the flight from London to Chicago (American Airlines Flight 99) suggested the patient arrived at O’Hara on the morning of April 24.
The attendance at the conference also raised the possibility that the HCW was treatment of patients in Jeddah, since the question asking about Jeddah was deleted from the released CDC transcript involved with in Jeddah, since the question asking about Jeddah was deleted from the released CDC transcript.
Detail on the conference attendance by the infected HCW would be useful.

Prior to being admitted to the hospital, the Indiana patient had extended face-to-face contact on April 25th with a business associate in Illinois. The two had another brief contact on April 26th.
Just in conclusion, our contact investigation indicates that there may have been some spread in the Indiana -- from the Indiana patient.
The above comments from the introduction and conclusion in prepared CDC presser remarks on the Riyadh health care worker (HCW), who traveled to Munster, Indiana via London and Chicago to attend a medical conference in Chicago (10th Annual Meeting of American Physician Scientists Association). When he went to the conference he had a 30-40 minute face to face discussion with a colleague (described as a “business associate” above) on the first full day of the conference and had a shorter, 10-20 minute discussion on the second day. The conference ended in the early afternoon of the next day, Sunday, April 27, when he drove back to Munster. The contact with the family in Munster on Sunday afternoon and evening, as well as Monday morning prior to his arrival at the Emergency Department at Community Hospital, represents the most at risk period for the Munster family contacts.
The above scenario is supported by the times of the flights, media reports indicating the he came to Chicago for a “conference” and he “went” to the family in Indiana on April 27 when his medical condition deteriorated. The dates and itinerary of the conference, in downtown Chicago about 30-45 minutes from Munster, match the travel descriptions of the CDC and Indiana Department of Health, as well as associated media reports. The full travel log prior to the conference includes British Airways Flight 262 from Riyadh, RUH, to London, LHR, followed by American Airlines Flight 99 from London, LHR, to Chicago, ORD with a scheduled arrive of 10:50 AM CDT on April 24. He then took a bus from O’Hara to Munster, Indiana, a Chicago suburb, and arrived in the early afternoon. He then drove to the conference that afternoon/evening for meet and greet events in the evening of April 24, or drove to the conference on April 25, where he had the face to face conversation with a colleague who was an Illinois resident. He had a briefer talk with his colleague on April 26, and then returned to Munster on the 27, when the conference ended and when his symptoms became more severe.
The risk to the family was greatest on Sunday and Monday, because he was more contagious due to more severe symptomns, and an antibody test indicated he had already infected his colleague on Friday and/or Saturday via casual contact that included a handshake.
Thus, it is likely that one of more family members were infected in Indiana, which also is also true for HCWs at Community Hospital. Although all of the contacts were PCR negative, including the colleague in Illinois, detectable antibodies take several weeks to develop, so it is likely that several Indiana contacts will sero-convert and test positive this week, as indicated in closing remarks.

We have sequenced partial spike protein genes from another 25 viruses, showing 100 percent sequence identity with above-mentioned genomes The above comments from a letter submitted by Christian Drosten to ProMED on April 26 suggest that the 25 cases linked to the partial spike sequences were infected by the same novel sub-clade represented by the 6 nearly full sequences (C7149 and C7770 from hospital A collected on April 3 and 7, respectively, as well as C7569 from hospital B collected on April 5, followed by Jeddah sequences C8826 and C9055 collected on April 12 and April 14 from hospital A and C, respectively, as well as Mecca (C9355) collected on April 15) uploaded to the Drosten website.
All six of these sequences were nearly identical, even though they originated from 4 different hospitals in two different cites (Jeddah and Mecca).
This novel sub-clade is defined by 9 polymorphisms that have never been reported in any human MERS sequence (listed below with 4 camel sequence isolated from Taif in late 2003). Two of the polymorphisms are in the Spike gene (C23804T and A23953G), and the later encodes Q833R, which has not been reported in any MERS sequence other than the six recently uploaded from Jeddah and Mecca. Both of these polymorphisms are located between the Spike receptor binding domain and the first heptad repeat (see figure 5 here) and therefore are almost certainly within the partial spike sequence that exactly matched the released sequences (which were all identical to each other in this region).
In addition to spike polymorphism Q833R, the novel sub-clade has a change in ORF8b, T28778A, which encodes L6Q, which also has never been reported in a MERS sequences. Moreover, ORF8b has another non-synonymous change, L40P, which has never been reported in a human MERS sequence.
In addition to the 9 polymorphisms not cited in any prior human MERS sequence, the novel sub-clade has two additional changes that are rarely seen in human MERS sequences. One is a synonymous change, C16174T, which has only been reported in one MERS sequence, Taif-2, while the other is non-synonymous and creates changes in two proteins, K60N in ORF8b and D126H in the overlapping N gene. Human MERS sequences with this change are limited to a sub-clade that emerged late in the Al Hasa outbreak and was detected in 4 patients who were almost certainly contacts of each other (AH19, AH26, AH27, AH28) based on matching collection dates for the latter three case (who would have been infected by the same index case, AH19).
Thus, as seen in the table below, the protein of ORF8b, which is 112 amino acids in length, has three changes in the first 60 positions (L6Q, L40P, and K60N), which may limit the function of the protein product which may be involved in regulating RNA levels. ORF8 was disrupted by the 29 nt deletion in SARS-CoV, which was associated with the dramatic spread of the virus after the incident at the Metropole Hotel on Feb 20-21, 2003 which led to major nosocomial outbreaks in Hong Kong, Singapore, Hanoi, and Toronto linked to patients who were guests or visitors on the ninth floor of the hotel (the index case was in room 911).
The presence of three changes on ORF 8b, and ORF8 disruption in SARS-CoV by the 29 nt deletion, raises serious pandemic concerns.

Novel Zaire Ebola Sub-Clade In Guinea and Sierra Leone Recombinomics Commentary 22:30 July 29, 2014 The Infectious Disease Initiative at the Broad Institute of MIT and Harvard, which is part of the Viral Hemorrhagic Fever Consortium, has released 84 complete or nearly complete Ebola sequences (at Genbank – EBOV_1 – EBOV_84 with accession numbers KM233035 – KM233118) from Sierra Leone patient collections between June 2 and June 12, 2014. Nigeria Index Matches Ebola Sub-Clade In Guinea & Sierra Leone Recombinomics Commentary 17:15 August 11, 2014 The lab in LUTH under Prof Omilabu received samples on July 22nd and the next day provided results of a pan-FILOVIRUS family diagnosis, that is evidence of presence of a virus belonging to the family of Ebola virus (including Marburg, Ebola-Zaire, Ebola-Sudan Bundibugyo virus, Reston virus, and Taï Forest virus).
Samples were also sent to Prof. Happi’s lab at the Redeemer’s University (RUN), late on July 23. The Happi team worked and tested and confirmed that the virus was the specific Ebola-Zaire type virus early on July 25, 2014. All these happened before confirmation came in from Dakar. Liberia Spike In Ebola Cases & Deaths Raises Concerns Recombinomics Commentary 18:15 August 15, 2014
The latest Ebola WHO update (reporting case between August 12-13) shows a dramatic jump in new cases in deaths (116 and 58, respectively). These numbers dwarf those from Liberia in the same period (27 and 14, respectively) and suggest that yesterday’s WHO announcement that “Staff at the outbreak sites see evidence that the numbers of reported cases and deaths vastly underestimate the magnitude of the outbreak” may have a significant relationship to the deteriorating situation in Liberia in general and Monrovia in particular.
Recent reports indicate that efforts to limit movement from heavily impacted rural regions in Liberia have not prevented flow of cases into Monrovia. Moreover, recent reports have cited bodies in the streets of two of Monrovia’s slums, West Point and Clara Town, which may represent a fraction of infected and/or fatal cases in these densely populated regions.
WHO has noted barriers to control, which included infections and deaths of a large number of health care workers and closing of hospitals, resulting in bed shortages and treatment limitations.
These recent developments raise concerns of a literal “death spiral”.
