It's so frustrating when this happens. Just this week I had an appointment with the specialist I see for my back. I was finally seen by his PA an hour and 45 mins after my appointment time. The doctor never saw me. I would have been outta there well before an hour and 45 minutes and I would not have returned.
Tales such as this make me profoundly grateful that I can pretty much avoid the medical profession other than for routine screenings. I'm 61 so it would be foolish to avoid those. It also makes me grateful to live in an area well-served by doctors and hospitals so I can go elsewhere if they don't respect my time.

I live in Houston and we have great medical facilities. If this was just a routine visit I probably would have left but I desperately needed an injection in my back to get me back up and running. I inherited some disc issues from my biological father and when it flares up, it takes me out for several days.

He is probably going concierge. I think he would have eventually but Obamacare has made doctors sell or go to concierge much sooner than they planned to.

"First appointment" isn't always the solution. I had a doctor who was late anyway. My favorite part was when the staff said "why does he take appointments this early -- he starts his day at the hospital and he know he can't get here before 10".
I was waiting in his office for 45 minutes, alone. I was contemplating blowing up all the rubber gloves and kicking them around.
There wasn't a charge for that appointment, but had there been; I'd have sent him a bill for my time equating to the amount of the charge.

Unfortunately, there is a shortage of specialists. My orthopedic is sometimes triple booked. A coworker of mine had to wait 2 weeks to see a physical therapist. He needed a specific treatment that not all PTs can do. I asked him about his procedure and asked my PT about it. She said there is a shortage of PTs who can perform that treatment. It will probably get worse.
